It could be the way you're steaming the milk. Try keeping the wand on the surface of the milk a bit longer.

The chocolate sauce iv been using always falls through the creme. The milk seems to steam just fine -- I'm thinking maybe the mouth of our cafe's cups are too big and it's stretching the espresso creme too thin. I've been using 2 shots in a 12oz though, should still form a thick enough layer to hold the sauce.
